Administrative 360 Feedback Tool:  This is a survey CSSU administrators use to solicit feedback from faculty regarding their performance as administrators.  To access this form electronically choose a Word format or a PDF fill-in form below.  If you are a CSSU faculty member using these forms electronically, please remember to complete the form, print it out and submit it to your administrator.  Simply filling it out on the website does not ensure that your administrator will receive it.  Word version  (Note: We're using a revised version - Spring '04.)
